Terms and Conditions:
1. A non-refundable deposit of £100.00 is required on submission of this form
2. The balance of any payments must be received fourteen days prior to the date for which the fountain is required
3. Payments can be made by cash or cheque, made payable to The Luxury Chocolate Fountain or via BACS to S/C 602477 Account No 18138403 The Luxury Chocolate Fountain held at Natwest
4. A booking will be provisionally held for seven days after this booking form is issued. If we do not receive the deposit and this form completed, the booking will be cancelled, unless previously agreed
5. Please note, no refund will be offered for any bookings cancelled within fourteen days of the event
6. A claim will be made against the customer for costs incurred as a result of any malicious or accidental damage caused
